Primary Health Physician – Community Health Centre

About the role

The Primary Health Physician will join TAIBU Community Health Centre’s Black‑Focused Interprofessional Primary Care Team to provide comprehensive primary health care to Black populations across the Greater Toronto Area and local residents of Malvern. The role emphasizes anti‑racist, equity‑based, and culturally affirming practices.

Key responsibilities

• Perform clinical assessments, diagnosis, treatment, health education, and appropriate follow‑up in accordance with the College of Physicians and Surgeons of Ontario standards.
• Provide holistic care that addresses social determinants of health, including health promotion, illness prevention, and counseling.
• Refer patients to specialists, allied health professionals, or community services as needed.
• Conduct medical crisis intervention, triage, and outbreak surveillance.
• Participate in home visits following Centre protocols.
• Collaborate with internal programs, external agencies, and multidisciplinary teams to improve client outcomes.
• Contribute to the development, implementation, monitoring, and evaluation of medical protocols and directives.
• Share on‑call duties with other physicians as designated.

Required profile

• Physician qualified to practice in Ontario, adhering to regulatory and statutory requirements.
• Commitment to delivering care within an anti‑racist/anti‑oppression framework.
• Ability to work collaboratively in an interprofessional primary care team.
