Pharmacist – Retail & Long-Term Care

About the role

As a Pharmacist at Custom Health, you will ensure safe and effective medication use for patients across Ontario. You will work in a retail‑pharmacy environment, support long‑term‑care facilities, and use advanced technology to dispense and ship prescriptions.

Key responsibilities

• Review and accurately dispense prescriptions in compliance with regulatory standards.
• Gather comprehensive medical histories, maintain medication profiles, and provide personalized counseling.
• Answer patient queries on drug safety, interactions, and proper usage.
• Deliver and set up Spencer adherence devices in patients’ homes.
• Manage prescription transfers, physician calls, and conduct drug utilization reviews.
• Maintain patient and financial records, generate reports from Adherenet software.
• Oversee drug procurement, dispensing, monitoring, and provide drug‑related information to staff.
• Supervise Pharmacy Technicians and ensure efficient workflow.
• Prepare and maintain automated dispensing machines for rapid, accurate medication dispensing.
• Assist with shipping orders and billing procedures.

Required profile

• Valid RPh licence in Ontario.
• Minimum 1 year of professional pharmacy experience in a retail setting.
• Proficiency with Kroll pharmacy software.
• Experience in long‑term‑care pharmacy and automation.
• Strong communication skills, especially with elderly patients.
• Excellent verbal and written communication.
