Pharmacist – Primary Care Team

About the role

The Department of Family Medicine’s Queen’s Family Health Team is seeking a pharmacist to join a multidisciplinary primary‑care clinic in Kingston. Reporting administratively to the Clinic Manager and clinically to the Lead Physician, the pharmacist will provide patient‑centred pharmaceutical care and support the team in delivering safe, evidence‑based medication therapy.

Key responsibilities

• Collaborate with the interdisciplinary team to develop therapeutic plans for patients with chronic conditions such as diabetes, hypertension, dyslipidemia, heart failure and atrial fibrillation.
• Participate in the design, implementation and evaluation of chronic‑disease and health‑promotion programs, including anticoagulation management and medication reconciliation.
• Communicate with hospital and community pharmacists to ensure seamless transitions of medication care across settings.
• Conduct medication assessments and counselling for older adults, patients on multiple drugs, and recently discharged individuals.
• Provide up‑to‑date drug information, guidelines and evidence‑based recommendations to clinicians, residents and other staff.
• Educate family‑medicine residents and learners on medication safety, reconciliation and best practices.
• Deliver individual or group patient education sessions on medication use and health‑issue management.

Required profile

• Licensed pharmacist registered with the appropriate provincial regulatory authority.
• Experience working in a primary‑care or community‑pharmacy setting is preferred.
• Strong communication and teamwork skills to collaborate with physicians, nurses and other health professionals.

Required skills
