About the role

Join Brant Community Healthcare System as a Pharmacist and contribute to high‑quality medication management across our two hospital sites in Brantford and Paris, Ontario. You will work within a collaborative, patient‑first environment that values safety, equity, and continuous improvement.

Key responsibilities

• Prepare, dispense, and verify prescription medications for inpatient and outpatient services.
• Provide drug information and counseling to patients, families, and healthcare professionals.
• Participate in medication safety initiatives, adverse drug reaction monitoring, and formulary management.
• Collaborate with interdisciplinary teams to optimize therapeutic outcomes.
• Support urgent care and transitional care services at The Willett site.

Required profile

• Registered Pharmacist with a valid license to practice in Ontario.
• Commitment to delivering patient‑centred care and upholding professional standards.
• Strong communication skills and ability to work effectively in a team‑based setting.

Required skills

• Knowledge of hospital pharmacy operations and safety protocols.
