Pharmacist – Community Hospital

About the role

We are seeking a licensed pharmacist to join Brant Community Healthcare System, a two‑site community hospital network serving Brantford and Paris, Ontario. The pharmacist will work within our acute‑care and urgent‑care settings to ensure safe, effective medication use for patients across the continuum of care.

Key responsibilities

• Dispense prescription medications and over‑the‑counter products in accordance with provincial regulations.
• Conduct medication reviews, provide patient counseling, and support medication therapy management.
• Collaborate with physicians, nurses, and allied health professionals to optimize drug therapy.
• Maintain accurate pharmacy records, manage inventory, and ensure proper storage of pharmaceuticals.
• Participate in urgent‑care and transitional‑care services at The Willett and Brantford General Hospital.

Required profile

• Current registration with the Ontario College of Pharmacists.
• Bachelor’s degree in Pharmacy (or equivalent).
• Strong communication and teamwork abilities.
• Commitment to patient safety and continuous learning.
