Pharmacist – Community Hospital

About the role

We are seeking a dedicated Pharmacist to join our team at Brant Community Healthcare System, serving patients across our Brantford and Paris hospitals. The role involves delivering safe, effective medication services within a collaborative, patient‑first environment.

Key responsibilities

• Dispense prescription medications accurately and efficiently for inpatient and outpatient services.
• Provide medication counseling and education to patients, families, and healthcare staff.
• Collaborate with physicians, nurses, and allied health professionals to optimise pharmacotherapy.
• Maintain pharmacy inventory, ensure compliance with regulatory standards, and support medication safety initiatives.
• Participate in urgent care and transitional care services as needed.

Required profile

• Registered Pharmacist with a valid Ontario licence.
• Pharmacy degree (BSc Pharm or equivalent) and relevant clinical experience.
• Strong commitment to patient safety, quality care, and continuous learning.

Required skills

• Knowledge of medication dispensing processes and pharmacy regulations.
• Ability to work effectively in an interdisciplinary healthcare team.
