Pharmacist – Clinical Coverage at Community Hospital

About the role

Join Brant Community Healthcare System as a full‑time pharmacist providing clinical pharmacy coverage to an assigned unit within a two‑site community hospital system in Ontario. You will work in a supportive, interdisciplinary environment focused on quality care and patient safety.

Key responsibilities

• Deliver clinical pharmacy services and medication management to the designated unit.
• Ensure safe and accurate dispensing of medications.
• Collaborate with physicians, nurses, and allied health professionals to optimise patient outcomes.
• Maintain up‑to‑date medication records and documentation.
• Provide patient counseling and education on medication use.
• Participate in quality‑improvement initiatives and safety programs.

Required profile

• Pharmacy degree and registration with the Ontario College of Pharmacists.
• Full‑time availability with no overnight or on‑call shift requirements.
• Strong commitment to patient‑centred care and teamwork.
