Part‑Time RPN/RN for Malton Clinic (Home Health)

About the role

VHA Home HealthCare is looking for a part‑time Registered Nurse (RN) or Registered Practical Nurse (RPN) to join the Malton Clinic in Mississauga. The clinic operates seven days a week, providing appointment‑based home health services to clients referred through Ontario Health at Home.

Key responsibilities

• Provide wound care, medical injections, drain and ostomy care, chemotherapy connection/disconnection, and diabetes management.
• Assess, monitor, and develop individualized care plans and coordinate services.
• Evaluate care plan interventions through ongoing reassessment.
• Maintain high‑quality patient care by identifying and addressing factors affecting practice autonomy.
• Document all client interactions according to College of Nurses of Ontario standards and VHA policies.
• Communicate changes in client condition to the nursing supervisor and case coordinator.

Required profile

• Current registration as an RN or RPN in Ontario.
• Ability to work flexible hours within the clinic’s 8 am‑8 pm schedule, seven days a week.
• Commitment to client‑centred, community‑based care.
• Professionalism, sound judgement, and accountability in client and family interactions.
