Part-Time Physician – Transitional Care Program

About the role

CANES Community Care is seeking a part‑time physician to provide medical oversight for its Transitional Care Program at the Brampton (38 beds) and Bolton (35 beds) sites. The role supports medically stable clients who need short‑term, community‑based care while awaiting discharge to their homes or the next level of care.

Key responsibilities

• Conduct weekly on‑site rounds at one or both locations.
• Review client updates prepared by nursing staff and provide virtual or telephone consultations as needed.
• Offer after‑hours support for urgent or emergency concerns.
• Assist with outbreak management when required.
• Perform client assessments, health evaluations, and medication reviews.
• Initiate and manage specialist referrals, order and review laboratory requisitions, and facilitate palliative‑care referrals.

Required profile

• Current registration with the College of Physicians and Surgeons of Ontario (CPSO).
• Strong communication skills in English; additional languages are a plus.
• Interest in geriatric and community‑based care.
• Prior healthcare experience is an asset.
