Nurse Practitioner – Primary Care (Part‑time)

About the role

Woolwich Community Health Centre is seeking a part‑time Nurse Practitioner to join its interdisciplinary primary‑care team at the Wellesley site. The role involves delivering comprehensive primary health care to a registered client caseload and leading community programs that reduce unattachment rates in rural Waterloo Region.

Key responsibilities

• Provide wellness care, assess, diagnose and manage episodic illnesses and chronic diseases.
• Deliver group‑based health education and community‑facing programming.
• Collaborate with family physicians, dietitians, physiotherapists, and other health‑team members.
• Work one evening per week and share on‑call duties with other clinicians.
• Conduct home visits as needed, using a personal vehicle.

Required profile

• BScN with Primary Health Care Nurse Practitioner certification, registered with the College of Nurses of Ontario (Extended Class).
• 2–5 years of primary‑care nursing experience.
• Commitment to interdisciplinary teamwork and flexibility for evenings/on‑call.
• Knowledge of health issues across the lifecycle; assets include geriatrics, palliative care, mental health, chronic disease management, maternal‑infant health, and rural practice.
• Valid driver’s licence and access to a car for home visits.

Required skills

• Strong communication, organizational and problem‑solving abilities.
• Proficiency with computers and electronic medical record systems.

What we offer

• Salary $121,556 – $131,893 per year ($62 – $68 per hour).
• 22.5 hours per week (0.6 FTE) on a 6‑month fixed‑term contract.
• 4 weeks paid vacation, 18 sick days and 10 professional‑development days (prorated).
• Enrollment in the Healthcare of Ontario Pension Plan (HOOPP).
