Nurse Case Manager

About the role

Cencora is seeking a registered Nurse to join its Innomar Strategies team in Canada. The Nurse Case Manager will support patients, caregivers, and health‑care providers by delivering information on patient support services, disease states, and therapies, while ensuring timely access to medication.

Key responsibilities

• Provide patients and providers with information on support programs, diseases, and therapies using manufacturer‑provided materials.
• Conduct assessments, document interactions, and maintain electronic patient records.
• Act as liaison for medication access, coordinate with reimbursement specialists, and explore funding options.
• Monitor therapy adherence, report adverse events, and follow case‑management protocols.
• Collaborate with physicians, pharmacies, infusion clinics, and patient advocacy groups.
• Participate in training sessions, pharmaceutical meetings, and conferences.
• Assist in onboarding new Nurse Case Managers and contribute feedback on processes.

Required profile

• Licensed Nurse (RN or RPN/LPN) with active registration in the province of employment.
• Diploma in health care or related field; Bachelor’s degree preferred.
• Minimum 2–5 years of experience in patient support, case management, or related clinical roles.

Required skills

• Proficiency with electronic patient record systems.
