Nurse Case Manager (18‑month contract)

About the role

The Nurse Case Manager will provide comprehensive support to patients, caregivers, and healthcare providers throughout the therapy journey. Working within a pharmaceutical patient‑support program, you will ensure timely access to medication and promote adherence to treatment plans.

Key responsibilities

• Provide information on patient‑support services, disease states, and therapies using manufacturer‑provided materials.
• Assist patients and caregivers with assessments related to support services and act as a liaison for medication access.
• Document all patient interactions in the electronic patient record and maintain up‑to‑date case files.
• Follow case‑management protocols, including data collection, follow‑up, therapy disruption monitoring, and adverse event reporting.
• Collaborate with reimbursement specialists and physicians to secure funding and explore alternative options.
• Coordinate logistics with healthcare providers, pharmacies, and infusion clinics.
• Participate in training sessions, pharmaceutical meetings, and patient advocacy events.
• Provide mentorship to new Nurse Case Managers and contribute to process improvements.

Required profile

• Licensed Nurse (RN or RPN/LPN) with active registration in the province of employment.
• Diploma in health care or related field; Bachelor’s degree preferred.
• 2–5 years of experience in healthcare, pharmaceutical, insurance, or related fields.
• Experience in case management, patient support, or similar roles is essential.

Required skills

• Proficiency with electronic patient record systems.
• Knowledge of adverse event reporting procedures.
