Intensive Case Manager

About the role

The Intensive Case Manager will support families and individuals experiencing homelessness in the Halton region. Working within a client‑centered, housing‑first framework, you will help clients move toward stable, permanent housing while providing essential intake and administrative services.

Key responsibilities

• Assist families and individuals in developing housing plans, including income maximisation, tax filing, ID acquisition and connection to community supports.
• Respond to referrals from community agencies, assess needs using acuity tools, and complete Social & Community Housing applications.
• Place clients into permanent leased units that match their needs and provide orientation to new neighbourhoods.
• Conduct regular home or shelter visits to deliver life‑skills coaching, budgeting assistance, crisis intervention and community integration support.
• Maintain communication with housing providers, landlords and partner agencies to ensure coordinated support.
• Report suspected child abuse to the appropriate child welfare agency and refer clients experiencing domestic violence to specialised programs.
• Support a 24‑hour intake shift each month and provide flexible coverage as needed.

Required profile

• Post‑secondary degree or diploma in Human Services or a related field.
• Eligibility to register with the Ontario College of Social Workers and Social Service Workers (OCSWSSW).
• Experience working with adults and families facing homelessness, mental health or substance‑use challenges.
• Ability to work flexible shifts, including evenings, weekends and occasional 24‑hour intake duties.
• Valid driver’s licence, reliable vehicle and willingness to travel as required.
