About the role

Join Brant Community Healthcare System as a Hospital Pharmacist and play a key part in delivering safe, effective medication therapy to patients across our Brantford and Paris sites. You will work within a collaborative, patient‑first environment that values excellence, equity, and continuous improvement.

Key responsibilities

• Accurately dispense prescription medications and over‑the‑counter products.
• Verify prescription orders, check for drug interactions, and ensure appropriate dosing.
• Provide medication counseling and education to patients and caregivers.
• Maintain inventory, manage drug shortages, and oversee controlled substance records.
• Collaborate with physicians, nurses, and allied health professionals to optimise therapeutic outcomes.
• Support urgent care and transitional care services at The Willett site.
• Participate in quality‑improvement initiatives and medication safety programs.

Required profile

• Registered Pharmacist with a valid Ontario licence.
• Pharmacy degree (BPharm, PharmD, or equivalent).
• Strong communication and interpersonal skills.
• Commitment to patient safety and continuous learning.
