Home Care Nurse

About the role

The Home Care Nurse provides comprehensive nursing services to clients in their homes, collaborating with a multidisciplinary health team to promote independence and high‑quality care. This position involves assessment, care planning, and coordination of community resources for individuals and families.

Key responsibilities

• Conduct nursing assessments, develop and update care plans, and follow treatment orders for new and continuing clients.
• Coordinate referrals, case management, and interdisciplinary palliative care as needed.
• Provide direct nursing tasks in the home, including diabetes and foot care, in line with Home and Community Care Program policies.
• Supervise Home Support Workers, deliver orientation, assign tasks, and manage performance.
• Maintain records, submit monthly reports, and participate in research, evaluation, and professional committees.
• Order supplies and equipment in collaboration with the NIHB program and maintain links with community clinics and health agencies.

Required profile

• Bachelor of Nursing degree and eligibility for registration with the College of Registered Nurses of Newfoundland and Labrador.
• Minimum two years of experience in acute care, diabetes care, or community health nursing.
• Current CPR certification or ability to be trained.
• Strong interpersonal, written, and oral communication skills; ability to work independently.
