Home Care Coordinator

About the role

Habitat Health is seeking a Home Care Coordinator to ensure participants receive personalized, clinical home‑care services that support their independence and well‑being. As a member of the Interdisciplinary Team, you will assess needs, develop care plans, and coordinate services for older adults enrolled in the PACE program.

Key responsibilities

• Conduct comprehensive in‑home assessments of participants’ care needs, functional status, and safety risks.
• Perform home safety evaluations and provide recommendations to promote independence and reduce hazards.
• Observe and demonstrate the use of adaptive equipment, confirming participants can safely perform tasks.
• Collaborate with participants, caregivers, and family members to identify care gaps and service needs.
• Participate actively in the Interdisciplinary Team to develop, implement, and update individualized care plans.
• Partner with clinical and non‑clinical staff—including rehabilitation, nutrition, and social services—to ensure a holistic understanding of participant needs.
• Coordinate home‑care services in alignment with each participant’s care plan and goals.
• Review caregiver task completion, flagging missed or declined services.
• Monitor weekly home‑care notes to detect service gaps, condition changes, and address grievances.
