Family Medicine Pharmacist – Clinical Team

About the role

Join a multidisciplinary family health team as a Family Medicine Pharmacist, delivering comprehensive medication management and patient care. You will work alongside physicians, nurses, social workers and specialist providers at the Sherman Health and Wellness Centre, supporting seniors, diabetes, menopause care and chronic pain patients.

Key responsibilities

• Provide expert medication advice to the care team and patients.
• Perform medication reconciliation and support deprescribing initiatives for high‑risk regimens.
• Counsel patients on therapy changes, safe medication use and adherence.
• Answer prescribing questions from clinicians and contribute to clinical decision‑making.
• Mentor pharmacy students, medical learners and other trainees.
• Identify medication‑related risks, evaluate clinical data and recommend tailored adjustments.

Required profile

• Registered pharmacist with a valid license in Ontario.
• Experience in primary care, community pharmacy or family medicine settings.
• Strong analytical abilities and attention to detail.
• Excellent communication skills with empathy for patients and colleagues.
• Resourceful, proactive and able to manage complex medication histories.
