About the role

The Crisis Case Manager provides mental health triage, assessment, and short‑term support to individuals experiencing a mental health crisis. Working within a multidisciplinary team in Bracebridge, the role focuses on stabilising clients, developing coping skills, and linking them to community resources.

Key responsibilities

• Conduct in‑person and virtual mental health triage, assessment, and crisis intervention.
• Develop safety plans and short‑term treatment plans using crisis theory and harm‑reduction approaches.
• Provide psycho‑education, advocacy, and empowerment to enhance client coping skills.
• Coordinate short‑term case management and referrals to internal and external agencies.
• Liaise with community partners such as police, hospitals, and social service agencies across Muskoka and Parry Sound.

Required profile

• Bachelor’s degree in a relevant human service or health science discipline (or equivalent combination of education and experience).
• At least one year of experience working with individuals with mental illness, developmental disability, or substance‑use issues.
• Experience in supportive counselling, case management, or crisis intervention.
• Demonstrated empathy, emotional intelligence, and conflict‑resolution skills.

Required skills

• Proficiency in crisis intervention and resolution techniques.
• Knowledge of community resources and ability to build networks with landlords and service providers.
• Training in crisis response (e.g., ASIST) is preferred.
