Community Visiting Nurse – Day Shift (RPN)

About the role

ParaMed Home Health Care is seeking a Registered Practical Nurse (RPN) to join its community nursing team on a day‑shift schedule. You will travel throughout Leamington, Kingsville and surrounding areas, providing in‑home care to patients of all ages and helping them stay comfortable at home.

Key responsibilities

• Conduct scheduled home visits and deliver nursing care within your scope of practice.
• Perform patient assessments using appropriate tools and techniques.
• Develop, monitor, and adjust individualized care plans.
• Maintain accurate and up‑to‑date patient records for each encounter.
• Collaborate with registered nurses for complex cases and provide on‑call coverage after hours.

Required profile

• Current registration with the CNO as a Registered Practical Nurse in good standing.
• Valid driver’s licence and reliable vehicle for travel to patient homes.
• Reliability, flexibility, and willingness to work evenings or on‑call as needed.
• Experience in community or home‑care settings is an asset.
