Community Registered Practical Nurse (RPN) – Home Care

About the role

Join a leading home‑care provider as a Registered Practical Nurse (RPN) delivering community‑based nursing care. You will conduct scheduled visits to support patients of all ages in their homes, helping them stay comfortable and safe for as long as possible.

Key responsibilities

• Participate in scheduled community visits to assess and meet patient needs within your scope of practice.
• Provide ethical, safe, and compassionate nursing care.
• Utilise appropriate assessment tools and techniques to evaluate individual patient requirements.
• Develop, execute, and continuously monitor care plans, adjusting them as needed.
• Engage patients through a collaborative approach, building rapport and trust.
• Maintain accurate and up‑to‑date professional patient records.

Required profile

• Current registration with the CNO as a Registered Practical Nurse in good standing.
• Valid driver’s licence and access to a reliable vehicle.
• Experience delivering community care (preferred).
• Reliability, flexibility, and willingness to travel within a designated area.
• Experience working with vulnerable populations and a passion for community impact.
• Availability for on‑call coverage, including after‑hours, to ensure patient safety.
