Community Mental Health Worker – Prairie Mountain Health

About the role

The Community Mental Health Worker (CMHW) provides comprehensive mental health services to residents of Prairie Mountain Health, working in both community and facility settings. As part of an interdisciplinary team, the CMHW acts as primary counsellor and case manager, supporting individuals in their recovery across home, work, and social environments.

Key responsibilities

• Engage clients and families to build therapeutic relationships, conduct assessments, and develop individualized intervention plans.
• Perform risk evaluations for self‑harm, violence, and mental status deterioration, and intervene in crisis situations.
• Provide evidence‑based counselling, coordinate referrals, and advocate for client needs with community agencies.
• Document assessments, interventions, and recommendations in compliance with PHIA and FIPPA legislation.

Required profile

• Baccalaureate degree in Psychiatric Nursing or Social Work (or equivalent).
• Current licensure, Class 5 driver’s licence, and access to a personal vehicle.
• Minimum two years of full‑time experience in a mental health care setting within the past five years.
• Commitment to recovery‑oriented practice, cultural diversity, and confidentiality.

Required skills

• Certification in Non‑Violent Crisis Intervention and Applied Suicide Intervention Skills Training (ASIST).
• Proficiency with Microsoft Office (Outlook, Word, Excel, Access, PowerPoint) and internet applications.
• Strong written and oral communication, leadership, and independent judgment.
