Case Manager

About the role

The Association de la Communauté Noire de Côte‑des‑Neiges is seeking a dedicated Case Manager to join its community services team in Montréal. You will work directly with individuals and families, providing support, advocacy, and coordinated services to improve their wellbeing.

Key responsibilities

• Assess client needs and develop individualized service plans.
• Maintain a large caseload while meeting tight deadlines and high workload demands.
• Coordinate with community partners, social service agencies, and internal staff to ensure seamless service delivery.
• Document case notes accurately and monitor progress toward client goals.
• Provide crisis intervention and referrals as needed.

Required profile

• College/CEGEP education in Family and Community Services, Social Work, or equivalent experience.
• 3 to less than 5 years of experience in community or social services.
• Ability to work under pressure, handle large workloads, and meet strict deadlines.
• Strong interpersonal, oral, and written communication skills in English and/or French.
• Demonstrated reliability, organization, and a proactive, ethical approach.

Required skills

• Proficiency in standard office software (e.g., word processing, spreadsheets).
