Ambulatory Care Pharmacist

About the role

The Ambulatory Care Pharmacist joins an outpatient, multidisciplinary healthcare team to improve patient outcomes through comprehensive medication management. The role focuses on removing access barriers, providing guideline‑directed therapy, and ensuring integrated, accessible health services for the community.

Key responsibilities

• Assess patients' pharmaceutical needs and deliver individualized pharmaceutical care.
• Identify and resolve medication access issues and coordinate care as needed.
• Build and maintain collaborative relationships with patients, physicians, and other healthcare providers.
• Stay current with therapy guidelines and advancements in ambulatory pharmacy practice.
• Provide counseling to patients, families, and healthcare professionals on drug therapy.
• Order, interpret, and monitor laboratory tests when required.
• Conduct medication reconciliations, monitor adherence, and intervene to prevent drug interactions or adverse events.
• Perform preventative services such as vaccinations and wellness screenings.
• Document all activities in the pharmacy computer system and electronic medical record.

Required profile

• Licensed pharmacist with experience in outpatient or ambulatory care settings.
• Strong clinical knowledge of medication therapy management and guideline‑directed treatment.
• Excellent communication skills for interacting with patients and multidisciplinary teams.
• Commitment to continuous learning and staying up‑to‑date with pharmacy practice standards.

Required skills

• Proficiency with pharmacy computer systems and electronic medical records.
• Ability to perform medication reconciliations and therapy monitoring.
• Knowledge of immunization administration and naloxone dispensing where permitted.
