Registered Practical Nurse (RPN/LPN) – Community Home Care

About the role

The Registered Practical Nurse (RPN/LPN) will support the delivery of high‑quality nursing care in community home‑care settings, ensuring compliance with provincial standards and internal policies. You will work closely with physicians, residents, and the care team to maintain safe and compassionate care for seniors.

Key responsibilities

• Provide care in accordance with established nursing standards, physicians’ orders, and organizational policies.
• Observe, record, and report vital signs, symptoms, and changes in residents’ conditions.
• Assist in patient assessments and develop, implement, review, and evaluate individualized care plans.
• Monitor health status, identify changes, and coordinate referrals to physicians as needed.
• Process and execute physicians’ orders promptly and accurately.

Required profile

• Current certificate of competence from an approved institution for the province.
• Registered or eligible for registration with the provincial nursing regulatory body.
• Previous managerial experience in long‑term care or community home‑care preferred.
• Experience working with cognitively impaired residents.
• Comprehensive knowledge of nursing practices and infection‑control protocols.
• Strong communication skills, both verbal and written.
• Excellent organizational, planning, and multi‑tasking abilities.
