Registered Nurse / Practical Nurse – North Nursing Clinic (Toronto)

About the role

The North Nursing Clinic, a partnership between Ontario Health at Home and VHA Home HealthCare, is seeking a full‑time Registered Nurse (RN) or Registered Practical Nurse (RPN) to deliver specialised community‑based care in Toronto. Working 12‑hour shifts from 8 am to 8 pm, you will provide direct nursing services, education and coaching to clients with diverse health needs.

Key responsibilities

• Assess, monitor and develop individualized care plans for clients receiving home health services.
• Coordinate and deliver nursing interventions such as wound care, intravenous therapy, catheter care, diabetes and ostomy management.
• Evaluate the effectiveness of care plans through ongoing reassessment and documentation.
• Collaborate with interdisciplinary team members to ensure high‑quality, client‑centred care.
• Maintain compliance with College of Nurses of Ontario standards, VHA policies and community health nursing best practices.

Required profile

• Current registration as an RN or RPN with the College of Nurses of Ontario.
• Commitment to client‑ and family‑centred care in a community setting.
• Ability to work independently, manage a flexible schedule and participate in a bi‑weekly weekend rotation.
• Strong communication and professional judgement.
