Registered Nurse - Intensive Care Unit

About the role

The Southwest Ontario Aboriginal Health Access Centre (SOAHAC) is seeking a full‑time Registered Nurse to join its interdisciplinary team at the London site. Reporting to the Integrated Care Manager, the RN will develop and adapt client‑centred care plans, deliver direct nursing care, and provide case management for Indigenous clients across community and hospital settings.

Key responsibilities

• Facilitate the creation, implementation, and evaluation of individualized care plans.
• Provide direct nursing care and conduct comprehensive client assessments.
• Coordinate case management services, helping clients navigate community and hospital resources.
• Collaborate with primary care providers, families, and integrated team members to set realistic goals.
• Travel regularly to surrounding First Nations communities and support other SOAHAC sites as needed.

Required profile

• Registered Nurse in good standing with the College of Nurses of Ontario.
• Minimum three years of experience, preferably in community or primary care.
• Experience working with Indigenous populations is an asset.
• Demonstrated ability to deliver trauma‑informed, culturally safe care.
• Strong time‑management and resource‑allocation skills.

Required skills

• Clinical and diagnostic reasoning aligned with evidence‑based guidelines.
• Knowledge of chronic disease management, geriatric and palliative nursing practices.
