Nurse Case Manager – Remote, Canada

About the role

This remote Nurse Case Manager position supports patients across Canada throughout their treatment journey. You will act as a liaison between patients, caregivers, healthcare providers, and support services to ensure timely access to therapies and coordinated care.

Key responsibilities

• Provide education and ongoing support to patients, caregivers, and healthcare professionals about therapies and disease states.
• Coordinate case‑management activities, including assessments, follow‑ups, documentation, and therapy adherence monitoring.
• Liaise with pharmacies, infusion or injection clinics, and providers to facilitate treatment access and resolve issues.
• Maintain accurate electronic patient records and comply with reporting protocols.
• Collaborate with reimbursement specialists to secure funding approvals and explore financial assistance options.
• Monitor and report adverse events, therapy disruptions, and product concerns per regulatory requirements.
• Participate in product training, patient meetings, and healthcare events as needed.
• Support onboarding and mentorship of new Nurse Case Managers and contribute to workflow improvements.

Required profile

• Active RN or RPN/LPN registration in the province of employment.
• Diploma or degree in nursing or a related field; bachelor’s degree preferred.
• 2‑5 years of experience in healthcare, pharmaceutical, insurance, hospital, or patient case‑management settings.
• Strong understanding of patient support programs, therapy coordination, and healthcare communication.
• Excellent interpersonal, organizational, and communication abilities.
