Hospital Pharmacist – Transitional Care Unit

About the role

The Ottawa Hospital is seeking a qualified Pharmacist to join the Transitional Care Unit. This temporary position runs until approximately May 2027 and involves providing comprehensive medication management across day, evening, night, and weekend shifts.

Key responsibilities

• Ensure safe and effective drug therapy for patients in the Transitional Care Unit.
• Supervise, coach, and mentor pharmacy staff and students.
• Collaborate with multidisciplinary teams to optimize medication regimens.
• Maintain accurate medication records and contribute to quality improvement initiatives.

Required profile

• Baccalaureate of Science in Pharmacy from a recognized university or equivalent qualification.
• Unrestricted Part A license registered with the Ontario College of Pharmacists.
• Working knowledge of modern hospital care and drug therapy.
• Strong written and verbal communication, teaching, and interpersonal skills.
• Eligibility to work in Canada.

Required skills

• Proficiency with word processing applications.
• Experience using spreadsheet software.
• Familiarity with database applications.
