Registered Practical Nurse – Community Home Care

About the role

Join ParaMed Home Health Care as a Registered Practical Nurse (RPN) providing scheduled community visits to support patients of all ages in their homes. You will work independently while receiving RN backup for complex cases, helping clients stay comfortable and safe at home.

Key responsibilities

• Conduct scheduled home visits and assess patient needs within your scope of practice.
• Develop, implement, and continuously monitor individualized care plans.
• Document patient encounters accurately and maintain up‑to‑date records.
• Collaborate with patients, families, and the health‑care team to build trust and promote therapeutic relationships.
• Provide on‑call coverage, including after‑hours, to ensure patient safety.

Required profile

• Current registration with the CNO as a Registered Practical Nurse in good standing.
• Valid driver’s licence and reliable vehicle for travel to patient residences.
• Flexibility, reliability, and willingness to travel within a designated area.
• Experience in community or home‑care nursing is an asset.
• Commitment to serving vulnerable populations and making a community impact.

• Proficiency in clinical assessment tools and techniques.
• Ability to execute and adjust care plans based on patient response.
• Strong documentation and record‑keeping skills.
