Clinical Consultant Pharmacist – GTA East

About the role

CareRx is seeking a Clinical Consultant Pharmacist to join its Greater Toronto Area team. The role focuses on delivering high‑quality pharmacy services to long‑term care, supportive living and other congregate communities, ensuring continuity of care through close collaboration with on‑site clinicians.

Key responsibilities

• Act as the primary liaison between long‑term care homes, retirement communities and CareRx Pharmacy.
• Travel to assigned facilities to build and maintain strong partnerships with leadership and clinical teams.
• Conduct comprehensive medication reviews for residents in serviced communities.
• Complete all required documentation and billing in compliance with MOHLTC, the Ontario College of Pharmacists and other regulatory standards.
• Provide education to residents and staff on health‑related topics, including MedsCheck services.
