Visiting Nurse (RPN/RN) – Home Health Care

About the role

Bloom Care Solutions is seeking a compassionate Visiting Nurse (RPN/RN) to provide high‑quality, person‑centred care in the Mississauga and Halton region. You will work independently in clients' homes, delivering clinical services and supporting patients and families throughout their care journey.

Key responsibilities

• Conduct initial and ongoing in‑home assessments of health status, mobility, cognition, and safety.
• Provide clinical care including medication administration, injections, IV therapy, wound, catheter, and ostomy management.
• Perform diagnostic procedures such as blood glucose and urine testing and manage equipment like oxygen, feeding tubes, and CPAP.
• Complete intake interviews, review referrals, medical records, and insurance eligibility.
• Educate patients and caregivers on medication use, wound care, symptom monitoring, and equipment handling.
• Coordinate care with physicians, pharmacies, and other providers to ensure continuity.
• Document all assessments, treatments, and observations accurately in the electronic health record.
• Follow infection control, safety, and privacy standards (PIPEDA) and support discharge planning.

Required profile

• Regulated health professional (RPN) with a minimum of 2 years experience.
• Current BLS certification and up‑to‑date immunizations.
• Willingness to travel within the Mississauga and Halton region.
• Police Criminal Record Check with Vulnerable Sector Screening and TB testing.
• Excellent communication, time‑management, confidentiality, and professionalism.
