Pharmacist – Casual (Hospital)

About the role

We are seeking a qualified Pharmacist to join the Pharmacy Department of a leading rehabilitation hospital in the Niagara Region. This casual, on‑site position provides direct, patient‑centred pharmaceutical care and supports safe drug distribution across inpatient and outpatient services.

Key responsibilities

• Deliver clinical pharmacy services and drug distribution to optimise patient outcomes.
• Conduct patient interviews, medication histories and reconciliation, including polypharmacy reviews.
• Perform order entry and verification using the Oracle Cerner Provider Order Entry (CPOE) system.
• Provide drug information, participate in quality‑improvement initiatives and report incidents or adverse events.
• Collaborate with multidisciplinary teams, educate staff and act as a clinical preceptor or tutor as needed.
• Complete other duties assigned by the Director of Pharmacy and Quality Improvement.

Required profile

• Bachelor of Science in Pharmacy (Pharm D considered an asset).
• Current registration with the Ontario College of Pharmacists (Part A).
• Previous hospital pharmacy experience preferred.
• Ability to work independently, prioritize a varied workload and teach staff.
• Strong written and verbal communication skills.
• Excellent attendance record and discipline‑free history.

Required skills

• Proficiency with Oracle Cerner Provider Order Entry (CPOE) system.
• Medication reconciliation and polypharmacy review.
• Drug information provision and therapeutic drug monitoring.
