Family Practitioner / General Practitioner

About the role

We are looking for a compassionate Family Physician to join a small team of two doctors providing comprehensive care to residents of a long-term care home. The position focuses on continuity of care and offers a supportive, team-based environment.

Key responsibilities

• Serve as the Most Responsible Physician (MRP) for an assigned panel of up to 26 residents.
• Provide primary medical care, including assessments, treatment plans, and follow-up.
• Collaborate with on-site staff and other physicians to ensure coordinated care.
• Document all patient interactions using the PointClickCare electronic medical record system.
• Participate in regular on-site visits, approximately half a day every other week.

Required profile

• Board-certified or eligible Family Medicine/General Practice physician.
• Interest in long-term care and continuity of care.
• Ability to work independently while integrating with a small multidisciplinary team.

Required skills

• Proficiency with the PointClickCare EMR platform.
