Clinical Consultant Pharmacist (Part‑Time) – GTA

About the role

CareRx is seeking a Clinical Consultant Pharmacist to join its Greater Toronto Area team on a part‑time (0.4 FTE) basis. The role focuses on delivering clinical pharmacy services to long‑term care, supportive living and other congregate communities, ensuring residents receive optimal medication management.

Key responsibilities

• Act as the primary liaison between long‑term care homes, retirement communities and CareRx Pharmacy.
• Travel to assigned facilities to build and maintain collaborative relationships with on‑site physicians, nursing staff and leadership teams.
• Conduct comprehensive medication reviews for residents in serviced communities.
• Complete all required documentation and billing in accordance with MOHLTC, the Ontario College of Pharmacists and other regulatory standards.
• Provide education to residents and staff on health‑related topics, including MedsCheck services.
